Allison Teliszczak is a Board Certified Family Nurse Practitioner with Full Practice Authority. She received her Bachelor of Science Degree from Illinois State University, her Master’s Degree from the University of Phoenix, and later completed the Family Nurse Practitioner Program at Lewis University in Oakbrook, IL.
Prior to coming to Family Medicine, Allison had a background in ER nursing and has worked in Primary Care for 5 years, also having a special interest in gynecology care.
She currently resides in Woodstock, IL, with her husband Bradley and daughters Leighton and Reese. She really enjoys spending time outdoors and socializing with family and friends. She is an advocate for health across the lifespan, providing holistic and personalized care through collaboration with her colleagues.
Her primary focus is on preventative care, disease management, and health maintenance. One of her professional goals is to educate her patients to have them play an active role in supporting their own overall wellness. Her hobbies are crochet and nature walks with her girls, and her interests include reading and yoga, time with friends, and wine!
Allison is excited to continue her journey in caring for members of this community, and to finally find a forever home with Family Medicine for McHenry County. We’re grateful to have Allison as part of our Family Medicine Team!
